If you are a foreign-trained nurse who wants to work in the Netherlands, the BIG-register language route is usually the direct path. Inburgering is a separate civic obligation, and completing it does not by itself satisfy the BIG language requirement. The two paths can overlap in study time, but they test different things at different levels.
What each path actually is
BIG registration is the professional register for healthcare workers in the Netherlands. To register, you need Dutch at B2 or B2+ depending on your profession. The BIG-register Language Test is the mandatory test for foreign-trained healthcare professionals seeking registration. It is in person and sits at B2 or C1 depending on your profession. If you are already close to B2, this is the target you are aiming at.
Inburgering is the civic integration obligation under the 2021 Civic Integration Act, in force from 1 January 2022. The standard route is B1, with exams in reading, listening, writing and speaking plus KNM. A municipality can allow A2 after substantial effort. Older obligations used A2. Naturalisation currently requires A2 in 2026, with a move to B1 planned but no confirmed start date. None of this is a nursing qualification, and none of it replaces the BIG language requirement.
The level gap is the main decision
B1 is the inburgering standard. B2 is the BIG standard for nursing. That is one full level on the CEFR scale, and the jump is real, especially in speaking and listening under pressure. If your Dutch is at A2 or low B1, starting with inburgering material can build a base. If you are already at B1 and need to work, going straight at B2 medical Dutch is usually faster.
Staatsexamen NT2 is the general benchmark. Programma I is B1, for work and vocational study. Programma II is B2, for higher education and many professional roles. A nurse aiming at BIG is effectively aiming at Programma II level, even if the BIG test is the one that counts.
Providers that fit the BIG route
The Medical Dutch - Maastricht University Academy course is online and aimed at healthcare professionals with foreign diplomas. It prepares for the BIG-register language requirement at B2 or B2+. It fits nurses who already have solid B1 and want structured medical vocabulary and consultation practice. It is less suited to someone still building basic grammar.
The BIG-register taaleis Nederlands listing covers the mandatory Dutch language requirement and testing pathway for foreign healthcare professionals. It is online and in person, at B2. This is the reference point to read before you book anything else, because it tells you what the register expects. It is not a teaching course, so do not treat it as your only preparation.
Babel Medical Dutch B2+ in Utrecht is a dedicated medical Dutch course that includes BIG-register language exam preparation. It runs online and in person at B2 and C1. It fits nurses who want a taught course with exam focus. It is a poor fit if you cannot commit to a scheduled course or if you are below B1.
Katja Verbruggen NT2 is a social account rather than a course. The teacher posts exam tips for Staatsexamen and Inburgering, plus professional vocabulary for healthcare and office Dutch. It fits self-directed learners who want free, regular input between lessons. It does not replace a course or a test, and it is not a BIG preparation programme.
Providers for the Flemish side
If you are heading to Flanders rather than the Netherlands, the certificates differ. CNaVT PMT tests advanced Dutch for medicine and healthcare study in Flanders, at B2 and C1, online and in person. CNaVT PTGE is the Belgian certificate for Dutch proficiency in healthcare and care-sector professions, at B2. Both fit nurses and care workers targeting Flemish employers or study programmes. Neither is the Dutch BIG route, so do not use them as a substitute if your goal is registration in the Netherlands.
